The Berkeley School Online Enrollment Instructions 2010-2011
Please note, these forms are for current and newly admitted TBS students. If you would like to download the forms necesssary to apply to TBS, please click here.
TBS is pleased to announce that, beginning this year, we are offering paperless electronic completion and return of most TBS enrollment forms. There are also required state Public Health forms for certain TBS students, which cannot be completed electronically; you must separately download and complete those forms and return them to TBS. Click on the set appropriate for your child to a) download any applicable state forms, and/or b) access the online enrollment forms, which start with the “begin e-signature process” page. Follow the instructions to establish an online signature, and then begin completing the enrollment forms, clicking to sign when prompted. Upon completion, a signed copy will automatically be sent both to TBS and to your email address for your records. During the month of July, please call 510-665-8800, ext. 110 if you encounter any problems with the online enrollment process.
Please click on the set appropriate for your child:
- SET ONE is for All Returning 1st through 6th and 8th grade students (NOTE: for Kindergarten and 7th grade students choose SET TWO).
- SET TWO is for New Kindergarten through 8th grade students and All returning Kindergarten and 7th grade students. There are also links to required public health forms to download, print, and complete.
- SET THREE is for ECC only (3- and 4-year-olds). There are also links to required public health forms to download, print, and complete.
- FINALLY, download the Checklist, to make certain that you have completed all necessary forms.
